Standard Plan

The Standard Plan is provided through Avesis Incorporated. Members receive their annual vision exam (retail value: $65 to $70) for a co-payment of $15. They receive "Avesis Pricing" on all eyewear purchases regardless of the frequency. This plan has a nationwide network of providers that includes many popular retail chains as well as independent providers.

The DentaQuest Vision Standard Plan is only offered in conjunction with DentaQuest's DHMO and PPO dental plans and is a function of the actual dental enrollment. There is no separate Employee Application form. The Standard Plan can be offered to any group of 2 or more.

	Vision Exam:   Co-payment: $15. An insured benefit offering a comprehensive vision exam covered in full every 12 months by a participating Avesis provider. This is a vision exam only and does not cover a separate contact lens professional fitting fee.     Lenses:   All lens styles are available at Avesis pricing.     Frames:   No restrictions, regardless of style, size or color.     Contact Lenses:   Members will not pay more than $105 - $200, depending on lens type. Contact lenses may not be available at all locations.     Accessories:   10% - 20% savings on accessories such as nonprescription sunglasses. Out of Plan Providers: Best value is available from Avesis providers. However, members may go to a non-Plan provider for a vision exam and be reimbursed $35.
