Diamond Plan

The Diamond Plan is provided through Avesis Incorporated. Each year, as part of the insured benefit, members receive a comprehensive vision exam and new lenses, if necessary, at a $10 co-payment each. Members may also receive new frames every two years, also at just a $10 co-payment. However, Avesis pricing ALWAYS applies with significant discounts off retail pricing - so you never run out of benefits. This plan has a nationwide network of providers that includes many popular retail chains as well as independent providers.

The DentaQuest Diamond Plan is available to groups who enroll five lives or more. The Diamond Plan can be offered along with DentaQuest DHMO or PPO products or on a stand-alone basis.

	Co-payments:   Vision Exam $10, Materials $10. An insured benefit offering a comprehensive vision exam covered in full every 12 months by a participating Avesis provider. This is a vision exam only and does not cover a separate contact lens professional fitting fee.     Lenses:   A choice of glass or plastic lenses in single, bifocal or trifocal vision. Oversize lenses and in all ranges of prescriptions. Covered every 12 months.     Frames:   Standard benefit includes any frame that retails for $100 to $150. (Costs are approximate only.) Members may select a frame over this amount and pay the difference at Avesis Pricing. Covered every 24 months.     Contact Lenses:   Members may apply a $130 allowance toward contact lenses in lieu of the frames & lenses benefit. Covered every 12 months.     Out of Plan Providers:   Best value is available from Avesis Providers. However, members may go to non-Plan providers, pay their provider, and be reimbursed as follows:     Vision exam: $35   Single vision:$25, Bifocal lenses:$40, Trifocal lenses:$50, Frames:$45, Contact lenses:$130 Reimbursement is provided upon receipt of copy of the membership ID card and the original full-itemized invoice.
