	Standard Plan Monthly Rates $ 1.75 Single and $ 3.50 Family Premier Plan Monthly Rates: $ 5.00 Single and $10.00 Family Diamond Plan Monthly Rates: $10.00 Single, $16.50 Single + 1 and $22.00 Family
