How do I get information about my benefits?
You will receive a copy of your subscriber benefits from your Benefit Administrator during enrollment. You may also call our Customer Service department for this information. If you are a Classic, Advantage, Companion, Select, or ORTHOSelect plan member, please contact us at: 1-800-879-0288.

If you are an Access, ACCESS ePPO, or Choice plan member, please contact us at: 1-800-334-6277.
