If I am a Classic or Advantage DHMO plan member and have a dental emergency out of the service area, how will I be reimbursed?
DentaQuest will reimburse you up to a maximum of $50 per occurrence, minus any member fees, for dental emergency treatment received outside of our service area. Please complete and submit the Out of Area Benefit Emergency Claim Form along with a full itemization of charges and receipts from the dentist who provided the emergency care. Please return this information to the attention of our Customer Service department for processing.
