DentaQuest provides managed care, PPO and traditional indemnity dental benefit programs to employers, unions, and associations located in Maryland and Washington, D.C. through DentaQuest Mid-Atlantic, Inc.; and to those located in Virginia through DentaQuest Virginia, Inc.

	Maryland Dental Practices The state of Maryland requires all medical and dental plans to use the Maryland Uniform Credentialing Form as the sole application for credentialing or recredentialing of physicians and dentists.
