How do I change an employee's participating dentist?
Your employee may change from one participating general dentist to another by contacting our Customer Service departments. Advantage, Classic, Companion or Select plan members should contact us at: 1-800-879-0288. Access, ACCESS ePPO, Choice or Prime member should contact us at: 1-800-334-6277.
