One of my employees has lost her ID card. How do I get a replacement for her? You can obtain a replacement ID card by completing our Contact Us Form or you may contact our Customer Service departments via telephone. If your employee is an Advantage, Classic, Companion or Select plan member, please contact us at: 1-800-879-0288. If your employee is an Access, ACCESS ePPO, Choice or Prime plan member, please contact us at: 1-800-334-6277. Please have the subscriber ID number (typically the social Security number) and group number handy.
